Person Centered Planning
Person Centered Planning is a crucial approach where all staff are trained on the individual’s plan goals for the year. This training ensures that the goals set are achievable for the individuals we serve. By having all staff members well-versed in the specific goals and aspirations of each individual, a more tailored and effective support system can be put in place. This customized approach helps in fostering a more holistic and empowering environment for the individuals, leading to better outcomes and overall satisfaction. With dedicated and knowledgeable staff members, the person-centered planning process becomes more seamless and impactful, ultimately enhancing the quality of care and support provided to each individual.
Residential Services
We understand the importance of proper medication management in maintaining the health and well-being of our residents. With a full-time Registered Nurse (RN) on staff, along with a dedicated medication administrator, we strive to ensure that each individual receives personalized care when it comes to their medication needs.
Our staff are highly trained to assist with Activities of Daily Living (ADLs) and are dedicated to promoting independence by helping individuals build these essential skills. We understand the importance of maintaining one’s independence in daily tasks such as bathing, dressing, and grooming. Our team is here to provide compassionate support and guidance to ensure that individuals can carry out these activities to the best of their abilities. With our expertise and personalized care, we strive to empower individuals in their daily routines and enhance their quality of life.
Our clinical support services are backed by our dedicated clinical director, who is equipped to provide personalized behavioral support to our clients. Whether you require assistance with managing challenging behaviors, implementing treatment plans, or exploring therapeutic interventions, our clinical director is here to guide you every step of the way. We understand the importance of individualized care and strive to create a supportive environment that promotes your well-being and growth. Trust our experienced team to provide the highest level of clinical support tailored to your unique needs.
Our client’s health is one of the many needs that we address as a team. We help with not only making medical/dental appointments, but we also arrange transportation both to and from. We monitor our clients’ care and arrange for the proper annual and biannual appointments to keep our clients healthy. There is contact between the medical team and the office following all appointments and any needs or concerns are addressed at that stage.

To start the intake process, a call with the clinical department is taken to discuss the needs of the individual. Then, the individual will view the home to see if they like it and meet their potential roommate. Next, Ubuntu Care will hold a team meeting with the individual, guardian and case manager to discuss anything else. Lastly, the individual will decide if they want to move to our services.
Houses in Ubuntu Care will serve two individuals
Ubuntu Care facilities have personnel who hold the CRMA certificate (Certified Residential Medication Aide), which is a state-authorized certification that authorizes them to administer medications to people getting home and community-based services.
